Work Setting: Primarily Onsite with occasional mobile/home visits based on participant needs
Job Summary
We are seeking an experienced Physical Therapist Assistant (PTA) to join our rehabilitation team in San Jose, CA. Working under the supervision of a licensed Physical Therapist, you will provide restorative and rehabilitative therapy services to primarily elderly participants in both the clinic and home settings. In addition to patient care, you will assist with rehabilitation administrative tasks such as scheduling, inventory management, and maintaining department logs.
Key Responsibilities
- Administer physical therapy treatment plans developed by the supervising Physical Therapist.
- Assist with patient assessments, including vital signs, range of motion, and strength testing.
- Deliver therapeutic exercises and interventions to improve mobility, balance, endurance, and independence with daily activities.
- Monitor patient progress and report any changes to the Physical Therapist.
- Educate participants, caregivers, and staff on therapeutic exercises and equipment use.
- Document treatments accurately and maintain timely clinical records.
- Coordinate participant scheduling, therapy visits, and billing support.
- Track and maintain rehabilitation equipment and inventory.
- Update temperature logs and perform other rehabilitation administrative duties.
- Collaborate with the interdisciplinary care team to provide high-quality patient care.
- Participate in quality improvement initiatives, staff meetings, and required training.
- Maintain patient confidentiality and comply with all company policies and procedures.
Required Qualifications
- Associate's Degree in Physical Therapist Assistant from an accredited program.
- Foreign PTA degrees must be evaluated and determined equivalent to a U.S. Physical Therapist Assistant degree.
- Active Physical Therapist Assistant license in the state of California.
- Minimum 2 years of PTA experience in a clinical setting, preferably working with frail or elderly patients.
- Experience providing rehabilitative care under the supervision of a Physical Therapist.
- Strong clinical documentation, organizational, and communication skills.
- Ability to work independently and manage priorities in a fast-paced environment.
- Experience working with diverse patient populations.

- Geriatric rehabilitation
- Home health or community-based therapy
- Skilled Nursing Facility (SNF)
- Outpatient rehabilitation
- PACE or senior care programs
Work Environment
- Monday through Friday schedule with no weekends.
- Majority of work will be onsite, with occasional travel to participant homes as needed.
- Collaborative multidisciplinary team focused on improving the independence and quality of life of older adults.
