Job Title: Social Worker 2
Location: Maryville, TN 37801
Duration: 48 weeks (August 3, 2026 – June 30, 2027)
Shift: M-F, 8A – 4:30P
Job Responsibilities:
- Conduct social needs screenings and assessments for patients referred by primary care providers.
- Assist patients with social determinants of health (SDOH), including housing, food insecurity, transportation, clothing, and access to community resources.
- Develop and maintain community resource directories with eligibility requirements and referral information.
- Collaborate with physicians, nurses, social workers, and community organizations to coordinate patient care.
- Perform case management, including discharge planning, follow-up care, and patient education.
- Obtain medical records from hospitals, specialists, and imaging centers.
- Assist patients in obtaining durable medical equipment through community resources.
- Identify and reconnect patients who are overdue for care or lost to follow-up.
- Coordinate specialty referrals and track appointments as needed.
- Help patients access affordable prescription medications through assistance programs, CoverRx, RxOutreach, and similar resources.
- Complete patient assistance applications and act as liaison with pharmaceutical assistance programs.
- Document patient interactions, assessments, referrals, and care plans in the electronic medical record.
- Provide patient education and connect patients with appropriate educational resources.
- Attend orientation, provider meetings, clinical meetings, Health Council meetings, and community outreach events.
- Identify barriers to healthcare access and recommend solutions to improve patient outcomes.
Requirements:
- Experience in social work, case management, or care coordination.
- Knowledge of Social Determinants of Health (SDOH) and community resource navigation.
- Experience with discharge planning and care coordination.
- Strong communication, documentation, and organizational skills.
- Ability to collaborate with interdisciplinary healthcare teams.
- Proficiency using computers and standard office equipment.
- Valid driver’s license and reliable personal vehicle for community visits (personal vehicle listed as required equipment).
Preferences:
- Experience working in a primary care or community health setting.
- Familiarity with patient assistance programs and low-cost prescription resources.
- Experience coordinating specialty referrals and community-based services.
- Knowledge of electronic medical records (EMR/EHR) documentation.
