Job Title: RN-Care Manager
Location: Chapel Hill, NC 27514
Duration: 13 weeks
Shift/Schedule: Days, Monday–Friday, 8:30 AM–5:00 PM; no weekends, holidays, or on-call
Job Responsibilities:
- Provide comprehensive patient assessment, care planning, implementation, and evaluation.
- Manage patient cases with a focus on quality, continuity of care, patient satisfaction, and cost effectiveness.
- Perform case management, utilization review, and discharge planning activities.
- Review daily worklists, prioritize patients, and identify new admissions.
- Complete and document assessments and care plans in Epic.
- Participate in daily care management touchpoints, CAPP meetings, and weekly Complex Care Meetings.
- Identify and address barriers to discharge and coordinate appropriate interventions.
- Collaborate with physicians, nurses, social workers, utilization managers, patients, families, and other interdisciplinary team members.
- Coordinate family meetings and provide education regarding community resources and post-acute services.
- Develop and modify discharge plans based on patient progress and care team input.
- Monitor observation patients and assess readiness for discharge.
- Identify authorization requirements for post-discharge services and coordinate referrals.
- Support complex/high-risk patients and follow up on outstanding care-management action items.
- Ensure patients and families understand and agree with discharge plans.
- Document care management activities accurately and according to departmental and regulatory guidelines.
Requirements:
- Minimum 2 years of healthcare experience as a Registered Nurse.
- Strong patient assessment and critical-thinking skills.
- Experience with case management, care management, or discharge planning in an acute-care hospital setting is strongly preferred/required by schedule notes.
- Experience caring for acutely ill patients with complex medical needs.
- Registered Nurse license valid in the State of North Carolina.
- Nursing Diploma, ADN, or ASN from an accredited nursing school required.
- BSN or MSN preferred.
- Epic experience required/preferred based on department expectations.
- Strong care coordination and interdisciplinary communication skills.
- Excellent organization, attention to detail, adaptability, and ability to manage frequent changes.
Preferences:
- Hospital-based acute-care Case Manager/Care Manager/Discharge Planner experience.
- Experience in a teaching/academic hospital environment.
- Experience with Epic and care coordination.
- Experience supporting complex medical/surgical populations, including cardiology, nephrology, trauma, orthopedics, vascular surgery, neurosurgery, GI surgery, urology, transplant, ICU, and stepdown patients.
