Job Title: Registered Nurse – Case manager | Patient Care Coordinator
Location: San Francisco, San Jose & Roseville, CA
Shifts: 11.00 PM to 07.30 AM PST
Duration: 4- 6 + Months (Possible Extension)
Pay: Best in Market(Stipend also available)
Requirements:

• Active California RN/compact license (Required).
• 1 years of RN case management experience in Acute Care/Hospital setting
• BLS – AHA
• Rotating weekends

Responsibilities:

• Manage and coordinate patient care plans, ensuring smooth transitions across care settings.
• Conduct assessments and collaborate with interdisciplinary teams for effective discharge planning.
• Monitor patient progress, provide education, and ensure compliance with clinical standards.
