Family Nurse Practitioner (FNP) / Nurse Practitioner – Primary Care
Duration: 12-Week Contract (Potential Extension)

Position Summary
We are seeking an experienced Family Nurse Practitioner (FNP) with recent Primary Care or Urgent Care experience to provide evidence-based, patient-centered care. The Nurse Practitioner will collaborate with a multidisciplinary clinical care team to assess, diagnose, treat, and manage acute and chronic health conditions while promoting preventive healthcare and wellness.
Key Responsibilities
- Provide comprehensive primary and family healthcare services.
- Perform patient assessments, diagnosis, treatment, and follow-up care.
- Develop and implement individualized treatment plans.
- Manage chronic disease conditions and preventive care.
- Conduct routine physical examinations and health screenings.
- Order, interpret, and review diagnostic tests.
- Educate patients on disease prevention, medications, and healthy lifestyle choices.
- Coordinate care with physicians and other healthcare professionals.
- Document patient encounters accurately in the Electronic Medical Record (EMR).
- Perform clinic administrative duties including:
- Opening and closing the clinic
- Inventory management
- Insurance verification
- Patient follow-up
- Payment collection
- Maintaining a clean and safe clinical environment
- Participate in weekend and holiday rotations as required.
Required Qualifications
- Master’s Degree from an accredited Family Nurse Practitioner (FNP) program.
- Current National Board Certification.
- Active Nurse Practitioner license in the state of practice.
- Minimum 1 year of recent Nurse Practitioner experience in Primary Care, Family Medicine, or Urgent Care (Required).
- Strong knowledge of chronic disease management and quality measures.
- Ability to practice independently with minimal supervision.
- Excellent communication, organizational, and problem-solving skills.
- Proficient with Electronic Medical Record (EMR) systems.
- Willingness to obtain multi-state licensure if required.
