Position Summary

• We are seeking a self-motivated, energetic, detail-oriented, highly organized, and tech-savvy Bilingual Case Management Coordinator to join our Case Management team.
• This role is responsible for assessing, planning, implementing, and coordinating case management services through both telephonic and face-to-face interactions.
• The Case Management Coordinator manages a caseload of supportive and medically complex members, ensuring appropriate healthcare services, care coordination, and improved health outcomes.

Required Qualifications

• Fluent Bilingual Spanish/English (reading, writing, and speaking)
• Bachelor’s degree required
• Social Work or related field preferred
• No nursing degrees
• Minimum 1 year of Case Management experience
• Ability to multitask, prioritize, and adapt in a fast-paced environment
• Strong verbal and written communication skills
• Proficient with Microsoft Office (Word, Excel, Outlook)

Experience Requirements

• Minimum 1 year of Case Management experience (Required)
• Case Management Certification (Preferred)
• Long-Term Care experience (Preferred)
• Managed Care experience (Preferred)
• Strong Microsoft Office skills, including Excel
• Fluent in Spanish and English

Education

• Bachelor’s Degree required
• Social Work or related field preferred
• Nursing degrees are not eligible
