Job Summary

We are seeking an experienced Registered Nurse (RN) Case Manager to join a leading healthcare facility in Woodland Hills, CA. The RN Case Manager will coordinate and manage patient care throughout the continuum, ensuring high-quality, cost-effective, and patient-centered care. This role requires strong clinical judgment, discharge planning expertise, and collaboration with interdisciplinary healthcare teams.

Responsibilities

• Coordinate patient care plans from admission through discharge.
• Perform comprehensive patient assessments and identify discharge needs.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to develop individualized care plans.
• Facilitate appropriate utilization of healthcare resources while maintaining quality patient outcomes.
• Coordinate referrals for home health, rehabilitation, skilled nursing facilities, and community services.
• Educate patients and families regarding treatment plans, discharge instructions, and available resources.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Ensure compliance with hospital policies, regulatory requirements, and case management standards.

Requirements

• Active California Registered Nurse (RN) License.
• Current BLS Certification (AHA).
• Minimum 2 years of recent Case Management experience in an acute care hospital.
• Previous experience with discharge planning, utilization review, and care coordination preferred.
• Strong communication, organizational, and critical thinking skills.
• Experience with EPIC or similar EMR systems is preferred.
