Job Tittle: RN Patient Care Coordinator
Location: Portland, OR – 97232
Schedule: 8 hrs/day, 40 hours/week
Shift: 4:00 PM – 12:30 AM
Schedule: Variable days, Sunday–Saturday, including holidays
Position Overview
We are seeking a Registered Nurse – Patient Care Coordinator (PCC) to support outpatient care coordination and patient management. The RN will work within a variable schedule, including weekends and holidays, and may be required to float to neighboring healthcare facilities based on operational needs.
Key Responsibilities
- Coordinate and manage patient care across the healthcare continuum.
- Perform case management and patient care coordination activities.
- Support patients with Medicare and Medicaid coverage and related care needs.
- Document patient care and coordination activities accurately in Epic.
- Collaborate with providers, care teams, and other healthcare professionals.
- Assist with care planning, follow-up, referrals, and transitions of care.
- Adapt to changing operational needs and float to designated neighboring locations when required.
- Provide safe, patient-centered care while maintaining compliance with organizational standards.
Required Qualifications
- Active Oregon RN license or compact RN license on file.
- BLS certification.
- Minimum 1 year of Case Management or Patient Care Coordinator (PCC) experience.
- Experience with Epic electronic health records.
- Experience working with Medicare and Medicaid.
- Ability to work variable schedules, including weekends and holidays.
- Willingness to float to at least one neighboring location.
Preferred Qualifications
- Previous Kaiser Permanente experience preferred.
- Strong care coordination, case management, and communication skills.
- Experience working in outpatient or clinical care coordination environments.
