Job Title: Care Coach 1
Location : 75-90% local travel in Highlands County area
Duration : 3 Months
Shift : Mon-Fri; 8AM – 5PM
Job Description:
- Assesses and evaluates members’ needs and requirements to achieve and/or maintain an optimal wellness state by guiding members and families toward, and facilitating interaction with, resources appropriate for the care and well-being of members.
- Work assignments are often straightforward and of moderate complexity.
- Employs a variety of strategies, approaches, and techniques to manage a member’s health issues.
- Identifies and resolves barriers that hinder effective care.
- Ensures the patient is progressing toward desired outcomes by continuously monitoring patient care through the use of assessments, data, conversations with the member, and active care planning.
- Understands professional concepts, standards, regulations, strategies, and operating standards within their own work area.
- Makes decisions regarding their own work approach and priorities and follows direction.
- Work is managed and often guided by precedent and/or documented procedures, regulations, and professional standards, with some interpretation.
- Will travel throughout the counties to members’ homes and facilities.
Required Qualifications:
- Bachelor’s degree
- 2 or more years of related experience
- Knowledge of Microsoft Office Word and Excel
- Exceptional verbal/written communication and interpersonal skills
- Ability to use a variety of electronic information applications/software programs
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
Preferred Qualifications:
- Master’s degree
- Prior experience with Medicare & Medicaid recipients
- Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
- Experience with health promotion, coaching and wellness
- Knowledge of community health and social service agencies and additional community resources
